DIAPER BANK REQUESTS

• Diapers are repackaged into sets of 10 diapers by size, and quantities may be limited based upon the current inventory. 

• Community Partners should use this form to request up a maximum of 300 diapers (30 sets) per month from the Emergency Diaper Bank. 

• Once your order has been approved, your requested products will be available for pick-up on the first and third Tuesday of each month between 1pm and 4pm at United Way of Greater Lorain County (642 Broadway, Lorain, OH 44052).
